The Collaborative Rounding in Long Term Care Project

We invite you to take part in a confidential survey intended to obtain feedback from the loved ones of residents about the quality of care and services provided at this home. The important information you provide will assist us in identifying areas for improvement. Your participation is voluntary and we hope you will provide us your perspective. Your answers will be confidential and combined with those of others for reporting purposes. Individual survey answers will not be shared with anyone, unless you identify that you want to be contacted. This survey should only take about 10 minutes to complete. Once the survey results are summarized and analyzed, we will share the key findings with you through general communique.
